• Conduct Elderly/Disability Waiver Assessments
• Provide specifics on Medication Management - intake, dosage, schedules and prescriptions management
• Assist in the Development of a Care Plan
• Homecare Initials and Monthly Discharge Assessments
• Assisting clients with recovery from outpatient services
